Making a referral

You can make a referral by post, phone, fax, email or by using the online form. Please use this Referral Form in all cases. If you have older versions of our forms please destroy them.

Online form

You can complete this form online then press 'Submit' at the bottom of the form when you are finished. It will then be sent directly to us.

Waiting List

We aim to allocate a support worker within 10 working days. When demand is high we may not be able to provide a support worker within this time. In which case we may operate a waiting list. The person being referred will be informed of the delay and given an indication of when a support worker is likely to be available, or signposted to alternative support organisations.

Data Protection

Personal data is subject to provisions of the Data Protection Act 1998. Data will not be shared with third parties unless we are required to do so by law. Data may be used to help us monitor our services. Further information will be given to you by your support worker, or see the privacy statement on this website..
