Secure Online Form
Referrals can be made online using our secure online form. Simply complete the form then press the 'Submit" button at the end of the form. Your referral will be sent to us by email and a confirmation copy will be sent to the email addrerss you included in the Agency details.
Other Forms
If you don't feel able to complete the form online, we have included a form you can download and print. You will need to complete this form by hand and you can either post it or fax it to us.
Data Protection
Personal data is subject to provisions of the Data Protection Act 1998. Data will not be shared with third parties unless we are required to do so by law. Data may be used to help us monitor our services. Further information will be given to you by your support worker, or see the privacy statement on this website. Waiting List
We aim to allocate a support worker within 10 working days. When demand is high we may not be able to provide a support worker within this time. In which case we may operate a waiting list. The person being referred will be informed of the delay and given an indication of when a support worker is likely to be available, or signposted to alternative support organisations.
